With Dementia and Alzheimer\u2019s, some daily activities in the\u00a0home and personal care may become difficult and unsafe.<\/p>\n

Caring for clients with Dementia and Alzheimer requires experience and patience because it may cause unintended change in behavior such as hallucinations, wandering, rummaging and sundowning. Our Care managers and caregivers understand that it is progressive, which means it changes over time, and our role as care managers and caregivers changes with it.<\/p>\n

With dementia, every client is uniquely different and we believe that clients shouldn\u2019t have to leave their home, to live better. Our Dementia units observe, train and measure the following for best possible outcomes when caring for clients with dementia and Alzheimer\u2019s<\/p>\n

Changes in communication and behavior<\/strong> – agitation and aggression, rummaging, wandering, hallucinations, delusions, or paranoia, sundowning, communication skills and sleep problems<\/p>\n

Every day Care<\/strong> – daily activities, meal preparation, medication reminders, managing medical appointments, bathing, grooming , personal hygiene and staying physically active<\/p>\n

Relationship <\/strong>– helping family and friends understand the disease<\/p>\n

Safety<\/strong> – Fall prevention, medication management, driving safety, natural disaster preparedness and home safety<\/p>\n

Caregiver health<\/strong> – Real time support and respite in place for rough nights and temp break. Emotional and physical support for caregivers, Dealing with Feelings of Frustration and Guilt.<\/p>\n

Advocacy <\/strong>– Reaching out to payee reps for Unpaid and unopened bills, looking out for possible signs of Abuse, such as physical and financials.<\/p>\n
